Souhrn: Finerenon je nový selektivní nesteroidní antagonista mineralokortikoidních receptorů. Inhibuje nadměrnou aktivaci těchto receptorů, která u pacientů s chronickým onemocněním ledvin (CKD) a kardiovaskulárním (KV) onemocněním spouští zánět a fibrózu a vede k progresi renálních a KV onemocnění. Doporučená cílová dávka je 20 mg finerenonu jednou denně podávaná per os. Nejčastějším nežádoucím účinkem finerenonu je hyperkalemie. Nevykazuje však antiandrogenní nežádoucí účinky. V programu klinických studií finerenon prokázal účinnost a příznivý bezpečnostní profil v léčbě chronické renální insuficience u diabetiků 2. typu.

Finerenone: new treatment options for patients with chronic kidney disease and type 2 diabetes
Summary: Finerenone is a new selective nonsteroidal mineralocorticoid receptor antagonist. It inhibits excessive activation of these receptors, which in patients with chronic kidney disease (CKD) and cardiovascular (CVD) disease triggers inflammation and fibrosis and leads to progression of renal and CV disease. The recommended target dose is 20 mg of finerenone once daily administered per person. The most common side effect of finerenone is hyperkalemia. However, it does not show antiandrogenic side effects. In a clinical trial program, finerenone demonstrated efficacy and a favorable safety profile in the treatment of chronic renal insufficiency in type 2 diabetics.
