Souhrn: Hypercholesterolemie patří mezi nejvýznamnější rizikové faktory kardiovaskulárních onemocnění. Nicméně navzdory poměrně široké paletě dostupných hypolipidemik v léčbě, resp. dosahovaní cílových hodnot LDL-cholesterolu, pokulháváme. Zatímco na začátku 21. století jsme měli k dispozici pouze statiny (tehdy ještě bez nejúčinnějšího z nich – rosuvastatinu) a fenofibrát, rozšířením portfolia hypolipidemické léčby o ezetimib a inhibitory PCSK9 lze významně zvýšit procento pacientů, kteří dosahují cílových hodnot LDL-cholesterolu, a tím redukovat jejich kardiovaskulární riziko. Bohužel přestože jsou obě tyto lékové skupiny v České republice dobře dostupné, nejsou dostatečně využívané. Další nadějnou léčebnou možností by mohl být inklisiran, který ale stále nemá v České republice úhradové podmínky, naopak zdá se, že již velmi brzy budeme mít k dispozici další účinné hypolipidemikum – kyselinu bempedoovou.

Lipid lowering therapy – proven treatment options and prospects
for the near future
Summary: Hypercholesterolemia is one of the greatest risk factors of cardiovascular diseases. Despite of the wide range of available lipid lowering treatment, we are still not successful enough in reaching of LDL-cholesterol targets. At the beginning of 21st century only statins (at that time without the most effective of the them – rosuvastatin) and fenofibrate were available. With expanding the lipid lowering treatment portfolio by ezetimibe and PCSK9 inhibitors, we are able to significantly increase the percentage of patients achieving LDL-cholesterol target value and thereby reduce their cardiovascular risk. Unfortunately, although both of these medications are commonly available in the Czech Republic, their use is insufficient. Another promising treatment could be inclisiran, but there are no reimbursement conditions from health insurance in the Czech Republic available yet, on the other hand, it seems, another lipid lowering agent – bempedoic acid, will be available soon.
