Souhrn: Kontinuální monitorace glykemie (CGM) je klíčovým zdrojem informací pro vedení léčby diabetu. Pro pacienty s diabetem 1. typu jsou data z CGM podkladem pro kvalifikované určení dávky inzulinu v reálném čase. Lékař na základě retrospektivního rozboru CGM určuje problematické body a stanovuje další strategii léčby. Při léčbě diabetu 1. typu je trvalé používání CGM považováno za standard léčby a v České republice je plně hrazeno formou ročního rozpočtu. Pacientům s diabetem 2. typu, sekundárním nebo gestačním diabetem je hrazena profesionální CGM 3× za rok. Analýza křivky pomáhá při výběru nové antidiabetické léčby nebo při úpravě dávkování léků stávajících. Pacient z otevřené monitorace profituje v oblasti úpravy stravovacích návyků a přizpůsobení fyzické aktivity vývoji glykemie. Odbornou společností byla navržena úhrada CGM pro pacienty s diabetem 2. typu, gestačním či sekundárním, kteří jsou léčeni intenzifikovaným inzulinovým režimem, neboť trvalé používání osobní kontinuální monitorace umožňuje podobný benefit jako u diabetu 1. typu.

Glucose sensors: support in the treatment of all types of diabetes
Summary: Continuous glycemic monitoring (CGM) is a key source of information for guiding diabetes management. For patients with type 1 diabetes, CGM data is the basis for qualified insulin dose determination in real time. Based on a retrospective CGM analysis, the doctor determines problematic points and determines the next treatment strategy. In the treatment of type 1 diabetes, the permanent use of CGM is considered the standard of treatment and is fully covered by the annual budget in the Czech Republic. Patients with type 2 diabetes, secondary or gestational diabetes are reimbursed for professional CGM 3x per year. Analysis of the curve helps in choosing a new antidiabetic treatment or in adjusting the dosage of existing drugs. The patient benefits from open monitoring in the area of adjustment of eating habits and adaptation of physical activity to the development of glycemia. The professional society has proposed reimbursement of CGM for patients with type 2, gestational or secondary diabetes who are treated with an intensified insulin regimen, as the permanent use of personal continuous monitoring allows a similar benefit as for type 1 diabetes.
